I am in network specifically for mental health with Premera/Lifewise, Regence, Aetna and First Choice Health. Sometimes another insurance plan will agree to cover visits with a specific case agreement, however they will most likely require you to pay out of pocket and reimburse you with a super bill from my office. Out of pocket costs for the first appointment are $275-$300 and follow up appointments range from $100-$260 per session.
Please call your insurance company before you make an appointment to ensure I am in network. Helpful questions to ask are:
*What are my mental health benefits?
*What is the coverage amount per therapy/medication management session?
*How many mental health sessions does my plan cover?
*Is Sarah Woodall, Psychiatric Nurse Practitioner within my network providers?
*How much does my insurance pay for an out-of-network provider?
*What is my deductible? (amount you have to pay before insurance benefits start to apply)
*What is my co-pay amount per visit?
*Is a referral required from my primary care clinician?
If you would like to schedule an appointment with me, I will need the following information:
Once you are scheduled I will send you an email invitation to the patient portal for my office. This invitation must be activated within 72 hours. Here you can create a login, go to the check in section and complete new patient forms prior to your first appointment.
At your first appointment bring your insurance card, copay, the names and doses of any medications or supplements you are currently taking, and the contact information for your Primary Care Provider.This information is best completed via new patient forms sent electronically via email and must be completed prior to your appointment.

- First name, last name, date of birth and current address
- Phone number and email safe for me to leave a message on
- Insurance company name, ID number (if you are using insurance)
